Support Co-ordination

If you are funded for Level 2 Support Co-ordination, we would love to assist you with our well-established approach to co-ordination.

The TriCollective model of Co-ordination relies heavily on planning and budget establishment when you first come to us, to ensure your plan funding is utilised efficiently to meet as many needs as possible.

Key moments in your plan duration are identified and allocated to prevent you from being without service when unexpected needs arise, or when you reach the end and its time for your review.

Your budget may or may not allow for frequent contact with your Co-ordinator, however we will work with you to ensure it does not leave you feeling alone or without support when you need it.

Our considered approach to establishing your Co-ordination needs include;

• Breaking your goals into actionable tasks

• Establishing your plan budget including a flexible budget to allow for unknowns or extras

• Gathering support services to deliver your preferred services

• Seeking out new and innovative approaches to utilising your plan so you get the best experiences from it

• Advice, Insight, Intervention and Direction as needed

• Collaboration between people and services in your life that impact your care and wellbeing

• Support and planning prior to NDIS review

• Attendance and Assistance for the Plan review Meeting

• Post review debrief

We provide oversight from a Senior Co-ordinator with almost ten years’ experience in Co-ordination team management and NDIS Support Co-ordination. Your co-ordinator also receives varied training opportunities, cross-role work experience and 1:1 supervision.’
